What to Expect on Your First Visit
- You will register with our friendly front office staff. They will help you navigate the insurance process and treatment program so you can focus on your recovery.
- You will meet with your physical therapist for the initial evaluation.
- During the evaluation your therapist will ask questions about your medical history, current pain, or injury. Your physical therapist will also answer any questions you may have about therapy and your condition. Open communication helps us get you better.
- Your movement, strength, coordination, and much more will be evaluated. Our purpose is to help you, so please communicate to your therapist if anything is hurting or bothering you during the examination.
- Your therapist will recommend and prepare a course of treatments to ensure the quickest recovery and the best results. Your therapist will discuss the recommendations with you.
- There will be some treatments performed the first day to get you started on your road to recovery. However, during the next visit you will get into the full swing of your treatment plan.
- You will schedule a series of visits with the front desk. The amount of therapy needed will be discussed with you, depending on the recommendations of your therapist and physician. We offer many convenient appointment times.
- Your therapist will write up their findings and treatment plan to submit to your referring physician or other medical professional who you choose.

Before Your First Visit
- Make a list of questions and concerns you would like to discuss with your therapist.
- List signs and symptoms that can be of importance to the therapist.
- Have in writing key past medical history, previous treatments, current medications, and other important information.
- Please call our office to verify your insurance benefits.
- Arrive 15 minutes early for your first appointment.
- Bring with you a copy of your insurance card and a photo ID.

Physical Therapy Plus accepts most private, Workers’ Compensation and No-Fault insurances. Click on the buttons above to see a list of the insurance companies with which we work. We also have self-payment options with affordable, customized payment plans for those who need it and accept MasterCard, VISA and American Express for your convenience. Please contact our billing department (315)-239-3779 if you wish to work out a customized payment plan or have any other questions.

PATIENT RESPONSIBILITY
If your insurance coverage includes co-payments, co-insurance or deductibles, it will be your responsibility to pay them at the time of service. Our billing staff will be happy to help you understand your benefits and financial responsibilities and help you in any way that they can. If the information you give us is incorrect or inaccurate, you may be held responsible for payment of services rendered. Therefore, it is very important to let us know immediately if your insurance changes so as to better assist you.
PRIVATE & NO FAULT INSURANCE
Please make sure you give us appropriate carrier information so we can process your claim quickly and accurately. We work with a wide variety of Private and No Fault carriers to make things as convenient as possible for our patients.
MEDICARE INSURANCE
Medicare patients, by law, are required to see their Primary Care Physician or Referring Provider on a monthly basis. (This is a Federal regulation.) To continue physical therapy at Physical Therapy Plus a plan of care will be submitted to your physician for his or her approval.
WORKERS’ COMPENSATION
Please check with your employer’s Workers’ Compensation Insurance carrier to make sure that you have completed the accident report and have filed all the required paperwork in order to collect Worker’s Compensation benefits. It is also important that you let your Physical Therapy Plus therapist know when you have a follow-up appointment with your referring physician. Our Physical Therapy Plus therapist will send your physician a progress note regarding your care at Physical Therapy Plus.

What is Physical Therapy?
Physical therapy is a type of treatment that helps you move better or gives you relief from pain. The goal of physical therapy is to preserve, enhance, or restore your movement, physical function and fitness level.
How long will I need physical therapy?
You will need physical therapy until you and your therapist decide you have reached the appropriate functional level. Your doctor may also monitor your progress.
What should I wear for my session at Physical Therapy Plus?
Wear or bring comfortable, non restrictive clothing to physical therapy so the therapist can evaluate the injured area and you can stretch more comfortably. Closed toe shoes, preferably sneakers, are recommended.
What should I bring with me?
Bring insurance information, a list of medications you are taking, a physician’s referral (if you have one) and any records you have of the injury. If you have already downloaded and completed Physical Therapy Plus New Patient Registration Form and Medical History Questionnaire, bring them with you. And please plan to arrive 15 minutes early for your first visit to Physical Therapy Plus to allow us to help you with your paperwork.
Will I see the same physical therapist each time?
We prefer that you see the same physical therapist for your physical therapy visits so that we can provide you with continuity and consistency of treatment. This also makes it easier to monitor your progress.
How long does each visit last at Physical Therapy Plus?
Your visit will usually be 30 – 60 minutes long.
How long before I see a difference?
Everyone is different, but how quickly you improve depends on the seriousness of the injury or problem, your level of commitment to treatment in the office and at home, and the continuity of your visits.
Will my insurance pay for this?
Most insurance companies pay for physical therapy, but some have limited coverage. You can call the benefits line on your insurance card to find out what your specific plan covers. We also offer direct payment plans, if needed, or can design an individual payment plan that works best for you.
Do I need a referral from my doctor to receive treatment at Physical Therapy Plus?
We do have Direct Access, which means it is not necessary for you to have a physician’s referral to receive treatment at Physical Therapy Plus. However, many insurance companies require a physician’s referral, so check with your insurance carrier first.
